The care coordinator can:
-Join clients on medical appointments
-Provide education and support to the client, their families and loved ones who may have questions
-Assist with barriers to medical care and treatment, including insurance coverage, ID, and basic documentation.

At HHHRC, we offer rapid, 20 minute Hepatitis C (HCV) testing, both at our office and through outreach. To learn more about testing options, click here. For persons that are rapid HCV positive, further evaluation and linkage to medical care are offered for free through Hep Free Hawaii.

If you are living with Hepatitis C, we invite you to learn more about our Hepatitis Care Coordination program. The Hepatitis Care Coordinator provides free care coordination support to anyone diagnosed with Hepatitis C to move towards medical care and eventual treatment.
